DUCHENNE MUSCULAR DYSTROPHY: Cellular, Tissue and Treatment Events
Resumo
A distrofia muscular de Duchenne afeta aproximadamente 1 a cada 3500 meninos nascidos vivos. A causa da doença é a ausência da distrofina: proteína do complexo distroglicano presente nas fibras musculares. Ao nascimento, não é possível observar os sintomas, porém a doença progride rapidamente necessitando do uso de cadeira de rodas. Mesmo com intervenções farmacológicas e médicas, o paciente sucumbe por complicações cardiorrespiratórias. A fisiopatologia da doença consiste em eventos celulares e teciduais. Ocorrem anormalidades celulares na homeostase de cálcio, na proteólise, no apoptose, no estresse oxidativo e na integridade da membrana. No tecido, as manifestações patológicas são disfunções vasculares, fibrose e infiltrado inflamatório no músculo. A fim de amenizar e atrasar os sintomas da doença, há uma grande variedade de tratamentos que são divididos em intervenções farmacológicas, terapias celulares e terapias gênicas. O presente trabalho visa descrever a fisiopatologia da doença e discutir os tratamentos disponíveis ou em desenvolvimento.
Abstract
The muscular dystrophy affects approximately 1 per 3500 live births boys. The cause of the disease is the lack of dystrophin protein: protein complex distroglicano present in the muscle fibers. At birth, it is not possible to observe the symptoms, but the disease progresses rapidly requiring the use of a wheelchair. Even with pharmacological and medical intervention, the patient succumbs to cardiorespiratory complications. The pathophysiology of the disease consists of cellular and tissue events. Occur cellular abnormalities in calcium homeostasis, proteolysis, apoptosis, oxidative stress and membrane integrity. In tissue, the pathological manifestations are vascular disorders, fibrosis and inflammatory infiltrates in the muscle. In order to minimize and delay the symptoms of the disease, there are a variety of treatments that are divided into pharmacological interventions, cell therapies and gene therapies. This paper aims to describe the pathophysiology of the disease and discuss the available and developing treatments.
